What’s the difference between under eye fat removal and fat realignment?
Both surgeries are for improvement of the eye shape, improvement of so-called dark circle, when lower eyelid is bulging and tear trough is deeply caved in.
Under eye fat removal is adequate when lower eyelid is bulging but tear trough is not deeply caved in, to remove the fat which is pushing the lower eyelid.
When tear trough is deeply caved in, fat removal may not be effective enough to improve the problem. Hence we need to lead the fat that is pushing the lower eyelid to actually fill the tear trough for an effective result, which is the under eye fat realignment surgery.
Dark circle surgery is done which is approached through the conjunctiva, when lower eyelid fats are protruded with deep tear trough lines below. The protruded lower eyelid fats should be pulled downwards and fixed to flatten the puffy skin.

2. Incise the conjunctiva inner part of the lower eyelids
3. Move the eye bag's fats down towards the tear trough and get them fixed
